If you live within our practice area of Goodmayes, you are welcome to register with the practice. Please attend with your NHS number/card and proof of address.
If you do not have an NHS number/card you will need to bring in your Passport as ID (including any current Visas) and proof of address. You will also have to fill out a new patient registration form and a health questionnaire. Please note incomplete or incorrectly filled forms may delay your registration with the practice. You can expect to hear about your registration within two weeks. Practice services are available to you only after your acceptance as a practice patient.
